Most advice about restaurants is written as though dinner were an exam. Order the grilled option, dressing on the side, skip the bread, and treat the whole evening as a thing to survive.

The measured data supports a calmer version, and it puts the blame somewhere other than the meal in front of you.

The short answer

Eating out adds roughly 200 kcal to a day, on average, once you account for the fact that people eat slightly less around it. One meal at that scale changes almost nothing about a week. What the research actually points at is frequency, portion sizes that are larger than anyone estimates, and the fact that restaurant meals tend to disappear from your own record of what you ate.

What eating out adds to a day

A 2014 analysis in Public Health Nutrition used individual level fixed effects regression across 12,528 US adults aged 20 to 64, each of whom completed two separate days of dietary recall in NHANES. Comparing a person against themselves on a restaurant day and a non restaurant day removes most of the confounding that plagues this kind of research.

Fast food consumption was associated with about 194 kcal of additional total daily intake. Full service restaurant consumption was associated with about 205 kcal. Saturated fat rose by two to three and a half grams. Sodium rose by roughly 296 mg on fast food days and 451 mg on full service days.

Note the direction of surprise here. Full service restaurants, the ones with tablecloths and a waiter, came out slightly worse than fast food on both energy and sodium.

What 157 measured meals contained

The intake study measures the net effect on a day. A separate study measured the meals themselves, and it did so without asking anyone to estimate anything.

A 2013 study in JAMA Internal Medicine took the 42 most frequently purchased meals from the nine most common restaurant categories, sampled 157 individual meals from randomly selected independent and small chain restaurants within fifteen miles of downtown Boston, and burned them in a bomb calorimeter to determine actual energy content.

The mean was 1,327 kcal per meal, roughly 66% of a typical day's energy requirement in a single sitting. Within meal variability was large, with an average standard deviation of 271 kcal, meaning the same dish from the same category could differ substantially between visits. And 7.6% of the meals exceeded 100% of a typical day's requirement on their own.

The comparison the authors drew is the useful one. These unlabelled meals averaged 49% more energy than popular meals from the largest national chains, and 19% more than national food database entries for equivalent items. The mental estimate people carry for a restaurant dish is calibrated on the chains that publish numbers, and the independent place down the road is not bound by that calibration.

Frequency is the variable, not the meal

Put the two studies together and the shape of the problem changes. A single meal of 1,327 kcal inside an otherwise normal week is absorbed without incident. The same meal three times a week, every week, is a different arithmetic entirely.

This is why the advice to order carefully misses. Ordering carefully optimises the wrong variable. Someone eating out twice a month can order whatever they want and it will not register. Someone eating out five times a week cannot order their way out of it, and the useful question for them is about the pattern rather than the plate.

Why restaurant meals vanish from your record

There is a second effect the calorimetry study implies without stating. When you cook, you handle every component. You know there were two tablespoons of oil because you poured them.

At a restaurant that information does not exist. The butter finishing the sauce, the oil the vegetables were tossed in, the sugar in the glaze: none of it reaches you, and none of it appears in whatever estimate you make afterwards. The 19% gap between the measured meals and the database entries for supposedly equivalent items is that invisible material showing up on a scale.

The consequence is not that you should stop going out. It is that a week containing four restaurant meals genuinely is harder for you to see than a week containing none, and if you are trying to work out why the trend moved, that is where the missing information is.

What actually works at the table

Two things, and neither involves interrogating a waiter.

The first is to get your protein early in the day rather than hoping the restaurant supplies it. Restaurant meals are reliably generous with energy and unreliable with protein, so a day built around an evening out is much easier if breakfast and lunch already did that work. The floor gets cleared before you sit down, and dinner becomes a meal rather than a calculation.

The second is to expect the scale to be wrong the next morning. A meal carrying an extra 400 mg of sodium will hold water, and the reading will show it. That is not fat. Treating it as fat is how a good evening turns into three days of compensating restriction, which is a far more expensive pattern than the dinner was.
