Not one thing, three, and they work through genuinely different pathways. The short answer

Sugar cravings mostly trace back to three separate mechanisms: a blood sugar swing from an earlier meal that primes reward circuitry in the brain, a stress response that shifts food preference toward sweet and high-fat options through cortisol, and short sleep that raises hunger hormones and specifically increases desire for sweet and starchy food. Any one of them can drive a craving on its own. Stack two or three together, a carb-heavy lunch, a stressful afternoon, a short night before it, and the pull toward something sweet compounds rather than adds.

The blood sugar and brain mechanism

A 2013 study in the American Journal of Clinical Nutrition used functional MRI to measure brain activity in men four hours after eating either a high-glycemic-index meal or a low-glycemic-index meal matched for calories and macronutrients. Lennerz and colleagues found the high-glycemic meal produced a sharper blood sugar spike followed by a steeper fall, and that the size of that fall correlated with greater activity in the nucleus accumbens, a brain region central to reward and craving, along with higher self-reported hunger. The low-glycemic meal, despite matched calories, didn't produce the same pattern.

This is a mechanism, not a moral failing. A blood sugar crash genuinely activates brain circuitry associated with wanting more of exactly the kind of food that caused the crash in the first place.

The stress and cortisol mechanism

A separate and much-cited 2001 study in Psychoneuroendocrinology exposed women to a laboratory stress task and measured their cortisol response, then offered them a range of snack foods afterward. Epel and colleagues found the women whose cortisol rose the most in response to the stressor ate significantly more sweet, high-fat food afterward than women with a smaller cortisol response, even though everyone faced the same task. Stress itself didn't predict the eating pattern nearly as well as the size of the cortisol spike did.

Which cause is most fixable

Of the three, the blood sugar mechanism is the most directly within your control day to day, since it responds to what and how much protein was in your last meal. Stress and sleep are real levers too, but they're slower to change and less directly tied to any single meal. Beyond that, treating a craving as information rather than a failure changes how the next hour goes. A craving that follows a light-protein lunch is telling you something useful about the lunch, not about your discipline.

When a craving does arrive, what you eat next matters more than whether you gave in. A protein-forward option tends to close the loop, since it addresses the blood sugar swing directly rather than adding to it.
