On average, most of the weight comes back within a year, gradually rather than all at once. In the STEP 1 extension, people who stopped semaglutide regained about two-thirds of the weight they had lost by one year later, and the improvements in heart and metabolic risk markers drifted back toward where they started. Semaglutide takes around 5 to 7 weeks to clear the body, so appetite returns over weeks, and the range between individuals was wide.
That is the average picture. Your own result depends a lot on what you have in place when the medicine wears off.
What the STEP 1 extension found a year after stopping
STEP 1 is the main trial behind Wegovy's approval. After 68 weeks, treatment stopped for everyone, including the lifestyle program that came with it, and an extension published in Diabetes, Obesity and Metabolism followed 327 of the participants for another year.
Those who had taken semaglutide had lost 17.3 percent of their body weight by week 68. By week 120 they had regained 11.6 percentage points of it, leaving them 5.6 percent below their starting weight. The authors summarise it as regaining two-thirds of the prior weight loss. Cardiometabolic improvements, gains in the heart and metabolic risk factors the trial tracked, reverted toward baseline for most of those measures.
Two details matter when you read this. The lifestyle support stopped at the same moment as the drug, so the extension tested stopping everything at once. And the extension analyses were exploratory, which the paper states plainly.
What STEP 4 showed about stopping earlier
STEP 4, published in JAMA, asked the same question in a controlled way. Everyone took semaglutide for 20 weeks and lost an average of 10.6 percent. Then participants were randomly assigned to continue or to switch to a placebo, without knowing which, and both groups kept the lifestyle support.
Over the following 48 weeks, the group that continued lost another 7.9 percent. The placebo group gained 6.9 percent. Waist size, systolic blood pressure and physical functioning scores all favoured the people who continued.
Taken together, the two trials say the same thing from different angles. Semaglutide holds weight down while you take it, and most of that effect leaves with it. For the regain biology itself, see why people regain weight after stopping a GLP-1.
How long Wegovy stays in your body
Wegovy's FDA prescribing information gives semaglutide an elimination half-life of about one week and says it will be present in the circulation for about 5 to 7 weeks after the last dose.
In practice that means nothing changes overnight. The amount in your body falls by about half each week, so the appetite effect fades over a month or two. Many people notice hunger and food thoughts returning in stages rather than on a single day, which is the window where a plan helps most.
Which side effects fade after stopping
The trials describe regain and health markers drifting back. They do not describe a withdrawal reaction in their published summaries. What tends to change for the better is the digestive side of things.
In STEP 1, nausea and diarrhea were the most common side effects and were usually mild to moderate and temporary. In STEP 4's withdrawal period, digestive events were reported by 49.1 percent of people who stayed on semaglutide and 26.1 percent of people switched to placebo. Fewer people off the drug reported them. We collected what people notice week by week in the side effects of stopping Ozempic, which applies to Wegovy too, since the active ingredient is the same.
Why food noise tends to return after stopping Wegovy
Semaglutide does more than make portions smaller. In a 12 week crossover trial, people on it reported less hunger, fewer food cravings and better control of eating than on placebo. A 20 week trial of the weight-management strength found the same pattern, with fewer and weaker cravings and 35 percent lower intake at a free-choice lunch.
Those effects belong to the medicine, so they fade as it clears. That is why so many people describe the return of food noise, the background chatter about the next snack, as the hardest part of stopping, harder than any physical symptom. Knowing it is coming, and having a few ways to quiet food noise ready, makes the first two months less of a shock.
Why the averages hide a wide range
The STEP 1 extension reports a standard deviation of 7.7 percentage points around that 11.6 point average regain. In plain terms, some people regained very little and some regained more than they had lost. An average of two-thirds does not mean everyone gets two-thirds.
The trials were not designed to explain who did better, so any list of reasons is informed guessing. The factors that come up again and again in maintenance research are protein, strength training, sleep, and catching small gains before they become large ones. Those are the same levers in how to maintain weight loss after Wegovy.
What you can watch from the first week
Because regain is gradual, the earliest signal is a small, steady rise in your average weight, well before clothes feel tighter. OffRamp's Regain Radar is built around that. It compares your 7 day average with your baseline and only calls drift when the average sits at least 2 percent above baseline for 7 days running, with an alert at 4 percent.
Alongside it sit two habits with the strongest case behind them: a daily protein floor set at 1.6 grams per kilogram of your goal weight, and two 20 minute strength sessions a week. None of it touches your medication. Whether, when and how you stop is a decision for you and your prescriber, and these seven questions can help you have that conversation.
